The following notices are provided for customers in accordance with applicable City of Dallas transportation regulations.

1. OPERATING AUTHORITY RATE INFORMATION
   All transportation services provided by this company are subject to rates established by the operating authority in accordance with Chapter 47A of the Dallas City Code.
   Rates are determined based on vehicle type, service level, distance, duration, and applicable fees, and are available to customers upon request.

   Fast Ice Inc provides transportation-for-hire services with pricing based on vehicle type, service duration, distance, and service requirements.

   Hourly rates typically start at:
   • Luxury Sedans: from $95/hour (minimum hours may apply)
   • Luxury SUVs: from $110/hour (minimum hours may apply)
   • Vans & Specialty Vehicles: rates vary by vehicle and service

   Flat-rate services (including airport transfers and point-to-point transportation) are available upon request. All rates are quoted prior to service and confirmed in writing.

2. ZERO-TOLERANCE POLICY FOR INTOXICATING SUBSTANCES
   This company enforces a strict zero-tolerance policy regarding the use of intoxicating substances.
   Drivers are strictly prohibited from operating a vehicle while under the influence of alcohol, illegal drugs, or any substance that may impair their ability to operate a vehicle safely.
   Any violation of this policy will result in immediate corrective action, including removal from service.

   Customers requiring wheelchair-accessible transportation are encouraged to request this service in advance to ensure availability.

5. HOW TO REPORT COMPLAINTS TO THE CITY OF DALLAS (ORD. 29696)
   Customers may report complaints regarding transportation services directly to the City of Dallas in accordance with Ordinance No. 29696.
